Adult Wellness Membership Plan
$350/Year
Adult Annual Membership is available for patients 15 years of age and older who do not have periodontal disease.

Two routine dental cleanings per year
Two check-up exams per year
Two fluoride treatments per year
Two oral cancer screenings per year
One annual wellness scan
Complimentary X-rays, including all routine intraoral X-rays. This includes panoramic X-rays every 3 years.
Discounted Services: Members receive a 20% discount on all dental procedures, including fillings, crowns, implants, teeth whitening, emergency dental exams, CBCT scans, and more.
No Waiting Periods: There are no waiting periods or pre-authorization requirements, no deductibles, and no maximums. You can begin using your membership benefits immediately upon enrollment.
Child Annual Membership Plan
$250/year
Our Child Annual Membership Plan is available for pediatric patients 14 years of age or younger.

Our Periodontal Maintenance Plan is available for patients needing periodontal disease treatment.

Family Bundling Options
Couple Wellness Plan: $650/year
Family Wellness Plan (up to 4 members): $1,000/year
Additional Family Members: $275/year (adult) and $175/year (child)

• Membership is valid for one year from the date of enrollment.
